VACATION  

Regular full-time eligible employees are able to earn and use vacation time.  The amount of paid vacation time employees will receive increases with the length of their employment according to the vacation schedule in Table 1.0.  Earned vacation that is not taken may not be rolled over from one calendar year to the next.

ADDITIONAL LEAVE  

 

In addition to vacation and holidays, HUVAL will also allow time off with or without pay to eligible employees for other various reasons.  The following section gives the definition and guidelines for using this additional leave.

                   Sick Leave

HUVAL provides paid sick leave benefits to all eligible employees for periods of temporary absence due to illness or injury. Regular full-time employees will accrue sick leave benefits at the rate of 3 days per year.

 

Unused sick leave will be allowed to roll over at the end of each year. However, the maximum accumulated sick leave shall not exceed 10 days.

Sick leave benefits are intended solely to provide income protection in the event of illness or injury. Terminating employees will not be paid for unused sick leave.

                   Personal Leave

 

HUVAL provides paid personal leave benefits to all eligible employees. Regular full-time employees will receive personal leave benefits at the rate of 2 days per year.

 

Personal leave is intended for the well being of the employee and/or his/her family. Personal leave is intended for be used for absences such as:

 

1.            Illness or injury to family

2.            Family reunions

3.            Children’s school functions

4.            Doctor’s appointments

5.            Bereavement leave

 

Unused personal leave will not be carried over at the end of the year. All personal leave not used on or before December 31st will be forfeited. Terminating employees will not be paid for unused personal leave.

GROUP HEALTH / DENTAL INSURANCE / LIFE AD & D POLICY

 

Huval & Associates, Inc., pays up to $150.00 towards each employee’s medical insurance, all the life insurance premiums and $10.00 on your dental coverage per month. The group health/dental/life insurance plan is a cafeteria plan. An Employee becomes available for coverage after thirty days of full time employment with Huval.  See attached Group Health Benefits Schedule and Dental Benefits Schedule for Coverage.

DENTAL

 

Deductibles:     $50 per person, per calendar year on basic and major services combined.

 

Calendar Year Maximum Benefits:     Preventive, Basic, & Major Services: A combined $1,250 per person.

 

Lifetime Maximum Benefit:     Orthodontia Services: $1,200 per eligible person.

 

Preventive Services: Waiting Period: None
Coinsurance: 100% of eligible expense

 

!                   Oral exams and prophylaxis (i.e., cleaning) (twice per calendar year).

!                   Topical fluoride treatment for dependent children up to age 16 (twice per calendar year).

!                   Full-mouth or panorex X-rays (one every 5 years).

!                   Bitewing X-rays (Up to age 13: twice per calendar year; Age 13 and above: once per calendar year).

!                   Periapical X-rays.

!                   One topical sealant for each unrestored permanent molar for dependent children, up to age 16.

 

Basic Services: Waiting Period: 3 months
  Coinsurance: 80% of eligible expenses after deductible

!                   Non-surgical extractions.

!                   Injection of antibiotic drugs (at time of initial treatment).

!                   Emergency exams.

!                   Palliative treatment for dental pain.

!                   Simple restorative services (i.e., fillings).

!                   Analgesia for up to age 13 (nitrous oxide gas).

!                   Stainless steel crowns on primary teeth.

!                   Space maintainers for permanently lost primary teeth for dependent children, up to age 13.

!                   Repair of recementing of crowns, inlays, bridgework or dentures.

!                   Relining of dentures (once every 36 months).

 

Orthodontia Services: Waiting Period: 24 months
  Deductible: None
  Coinsurance: 50% of eligible expenses per eligible person

Treatment necessary for proper alignment of teeth for dependent children.  Work to be completed prior to age 19 (may vary by state).

401 K GROUP RETIREMENT PLAN

 

Once an employee has reached one year of service and is over the age of twenty-one they become eligible for 401K contributions.  Contributions can be given up to 15% of your gross income but not less than 2% of your gross income.  HUVAL will contribute 25%/$1.00 up to 6% of your contributions.  HUVAL's contributions are adjustable each year upon the discretion of the principals.
